Edit 2 (2016-12-12): Ok, since the thread got sticky I think it needs major overhaul, I will try to keep it up to date with new info from other players threads and updating as we go, suggestions are welcome and help is appreciated.
I'm thinking to add which mission the particular achievement can be obtained, also moderators if you're free, feel free to update the post as well. Thank you in advance.

A thief's beginning - Pickpocket someone with Yuki.
Atheist Ox - Make the ox kick the statue at Sunpu Castle.
Chicken Dinner - Kill 11 chickens in one level.
Chunin - Kill 1000 guards.
Complete Mastery - Collect all badges.
Disguised - Put on Aiko's disguise.
Five Shadows - Execute a plan with actions for all 5 characters.
Flying Rocks - Kill a guard by throwing a rock with Mugen.
Gear up - Reclaim all your equipment on your second visit to Mount Tsuru.
Genin - Kill 500 guards.
Happy little accidents - Cause 3 accidents on the Nakasendō road.
Happy Planning - Kill 3 guards with the planks hanging from the Yagura Mon.
High Five the Moon - Climb the pagoda of the small temple at the Myōgi Pass.
Ice Ice Yuki - Kill a samurai with an icicle.
It's raining men - Knock out a guard by throwing another guard at them.
Jonin - Kill 50 samurai.
Merciful - Knock out 250 guards or samurai.
Meticulous - Hide 750 bodies.

Mugen Unchained - Free Mugen in Suganuma village.
No Gimmicks - Use Hayato's stone to rescue Mugen in Ōsaka Castle.

Old Man - Visit Takuma in his tower in Ōsaka Castle.

Tanuki Time - Run 500 meters with Kuma.
The Cave - Find the smiling creature in a little cave on the Nakasendō.
The End - Shoot Kage-sama with Takuma in Kage-sama's hideout.
The Old Way - Kill General Okkoto with an explosive barrel.
Thorough - Hide 250 bodies.
Time Paradox - Kill General Okkoto prematurely in Hida village.
Too Early - Get shot while carrying an explosive barrel.
Undeserved Honor - Let Lord Noboru commit Seppuku.
Warzone - Shoot an enemy inside a warzone or a shooting range.
When Cut Across the Neck - Your technique is magnificent.you have to use Aiko's blinding powder on one of the marksman (I chose the developer) and he‘ll misshot Noboru.wynik27's quote and his solution
Winner Winner - Kill a chicken.
Yabu - Kill Lord Yabu in his estate.

There are Achievements and badges. There is one avhiebment for completing all badges but there are many others for all sorts of things.

I guess then, there are some achievements you can get on pure luck or just looking them up on here if you really hell bend to get it. I bet eventually somebody will write achievements guide :).

FYI the game is really good, i love the character interactions, few times i lol'ed.

Time Paradox
Kill the the general in mission 6 (Hida Village, I think Okkotu is his name)

No Gimmicks
Use Hayato's stone throw skill instead of the mechanism to distract the guards to help Mugen in mission 1.

Question about "The Cave": where is that smiling creature? I killed everyone on the map so that I could look for him in peace, but I can't figure it out. Does it have anything to do with the yellow monk? (The characters make an observation of him, but other than that there doesn't seem to be a particular reason why he exists in the first place). Or do I just have to look for a particular crevice? Any hints will be appreciated!

Right where you find Yuki, on the wall to the left. There is a small opening in the wall, walk inside it.

It's a quote from the 1980 film "Shogun Assassin". Personally, I think this achievement has to do with the wooden training targets you can find in certain missions. Mugen can attack them with his katana, so you must need to track them all down.

I can confirm those lines are said by Noboru in the final mission, perhaps some special killing method will trigger this achievement.
