Try looking into the game folder and installing stuff from Redist directory.

Thanks!

Fixed by installing the DX in the Redist folder, weird the installation didn't happen when I first launched the game

Have you tried right clicking on the Exe and run Troubleshooter on it? I have it on Windows 10 retail and its working great. Not sure what the issue is. Silly question, did you verify it after download with Steam via Steam>SS-FEHD>Verify Game Cache? Best of luck.

We noticed lately that Windows decides for some of those older games to turn on Compatibility Options for the exe file all by itself. Which is bad, because the game is perfectly forward-compatible, and doing that breaks it. Plainly ticking that option off usually makes it work.

I've tried everything, compatibility, no compatability, windowed, full screen, redistx2. The game starts but refuses to load a mission, just an empty window.

I'm running W10/64 pro, perhaps the title runs 32 bit only?

No, it definitely runs normally on Win10 64-bit. I just tested it.
If it loads up the game, but doesn't want to start a level, then it's something else than what OP experienced. Please create your own thread and post SamHD.log file there. Thanks!
