I've been wanting to play through this remastered version since release but have been waiting for that to be patched out so I don't find myself halfway through the hardest difficulty playthrough only to find I wasted my time and can't get the achievement.
Sorry I merged two achievements in my head. The vita chamber one and the hardest difficulty one.
The game probably remembers that you never respawned with a vita chamber.
The resetting difficulty glitch would only affect the difficulty achievement. Did you complete the game on Survivor difficulty in more than one session and get the achievement?
Fortunately they carefully worded the achievement, mentioning that you will get it for meeting the requirements "without using a vita-chamber" rather than without dying.
If like me you want to be sure of it though, simply start a new game on Survivor, pause, and load up your Suvivor save file from THERE and it will remain on Survivor mode.
For the record, turning vita-chambers off simply means you're sent back to the menu when you die. Really it's better to leave them on and just reload your game after respawning, that way if you decide the challenge is too much for you, you can just say "screw it", not reload and just leave the vita chamber and continue your game without the achievement.
