No Death achievements are the ultimate test of skill and nerves, and I think having them in the game makes perfect sense given the nature of the game. They add countless hours of replayability if you're up for the challenge. Getting them isn't as simple as you make it seem because you need to consecutively beat all the levels in a chapter without dying in one sitting. You should give them a try.
But A+'s aren't exactly the easiest to get either, though. I dunno. While I feel better about the not-dying achievements, I still feel like there should be something for A+'s, or at least Dark World A+'s.
There's a time limit for each chapter that you need to get under but I'm not exactly sure what they are . But the limit is higher for the tougher chapters, for example in Rapture it is much higher than in The Forest etc. I'm sure you get the point. While they're not achievements for A-ranking all the levels per se, I personally think they're quite close enough even though they don't cover The End and Cotton Alley.
