I think that might be it, since I noticed the same thing recently. I tried shooting the rainbow beam to see what would happen if I didn't have sufficient en, and I think that made my en count disappear due to not having any. Very low amounts of en seem to hide it as well (I also bought a bunch of things and ended up with 9 en, which wasn't showing up).

On a similar note, the attack from the boss in this screenshot is also hurting when it shouldn't (I edited it a bit to avoid spoiling which boss it is, but you'll know which one it is if you've seen it):

On another similar note, the final chapter's ( _____ in Wonderland) boss's second attack (the one after the massive explosion) will cause damage if you're on top of where the stars spawn in the background. Probably applies to other background-started attacks used by this boss too. I think some slight reworks need to be done for attacks like this and the previous boss mentioned... it seems like any bullets that have a harmless state will always cause damage in the very first frame if you happen to be on the place where the bullet spawns.
Seana's attack buff says it costs 2500, but costs 3750.
Unsure about other characters.
#1: Red beam charge attacks sometimes tick the damage at a far higher rate than normal. Not sure why this happens, or if it's intentional, but it seems to happen randomly, as if there are some frames where highly rapid damage can be dealt with charged red shots if fired at the correct time. Also potentially worth noting, normal-speed ticks don't do damage to stunned enemies, but high-speed ticks do.
#2: Yellow beams seem to have a "sweet spot" where it'll end up doing damage at about twice the speed. The distance you have to be for this to happen is just a tad further than the distance between Erina and the edge of the screen. Easiest way to find this "sweet spot" is at Plurkwood where that other training dummy thing is, which is in the same room as the one you teleport into. Need to re-enter the room from the side though since teleporting into any area despawns all enemies in that area. Just beam the dummy from the farthest-away edge of either the teleporter, or the healing area, and that's where the "sweet spot" is. You can tell when you're in the right place by the increased hit sounds and damage numbers.
#3: Nothing complicated about this one. Basic rainbow beam attacks use up Sliding Powder's next-attack damage boost.
#4: Numb, Speed Down and Unstable (in the slow portion) prevent Ribbon from using her basic attacks (yellow still works, though). You can still use charged shots just fine, but it seems like Numb and any debuff that slows your attack speed is the problem. Giant only slows movement speed, so Ribbon can still use her basic attacks when affected by that. Being in a purple charge shot also still allows Ribbon to shoot (probably because it isn't directly a buff/debuff), but it makes the shots act a little funny (shoots a lot at once)... Are you supposed to be affected by your own purple shots in the first place though...? I mean it's great for hugely increasing your invulnerability frames and/or fire orbs by standing in them, but still...
And lastly, #5: Just found out about this one while continuing to test Ribbon things; the Defense Up buff is actually a 33.3% reduction rather than 25% like it says in the buff/debuff menu. Doesn't matter where the buff comes from; blue beam boost, Rita's town buff, or when bosses get the buff on very low health on Hell/Bunny Extinction, it's a 1/3 reduction in all cases. For the record, Attack Down/Up, and Defense Down are all working at their stated levels of 25%.
P.S. The tick rate of red charge shots is tied to the shot itself rather than an enemy. So if something wanders into (or out of and then back into) a normal tick rate shot, it'll always only get damaged at the normal speed, and high tick rate shots will always only damage at the high tick rate.
P.P.S. I've been aware of the purple shot tactic for a while now after accidentally utilizing it at some point a long time ago, but never actually tried to purposely use it until just now. XD It was against SP Ashuri (which I also tried fighting for the first time; I'm playing on Bunny Extinction and hoo boy is it intense), and I found a bug with the fight in the process: Survival badge doesn't work, even if you're not affected by the No Badges debuff at the time of the lethal blow. Seems like No Badges takes priority over applying the Endurance buff.

I found this out from SomeGuy712x's screenshots.
Yeah, I found it interesting that I was able to get up there, and I wonder if that colorful tile room was meant to be used for something at some point.
However, this seems to be a harmless glitch, since you can escape that off-map area by sliding in the lower-right corner of the empty screens to the right of the colorful tile room. Oh, and also, going off-map on the right side (from your "Out of map spot #1" location) just causes you to fall down to the save point next to where the shoot-em-up Lilith battle takes place.
So, maybe this glitch doesn't need to be removed, and could be left in as a fun little thing (and perhaps stuff could be added involving that colorful tile room in the future, making this glitch into an official "feature").

I also lost quite a bit of time because I did the Broke the Game skip right after Syaro's fight so that I wouldn't have to revisit the area later and the enemy levels jumped up a lot after finishing the red area(level 49 despite being just over 20 % items at the time). Maybe the level boost on enemies wouldn't be an issue on a run where you'd collect a lot more items before heading to System Interior.
Edit: This problem is gone now that I switched from Force DirectX 9.0/Windows XP Mode to Force DirectX 9.0c/Windows XP Mode in the launch options but, now, every time I close the game, the computer thinks it's still on so I have to use Task manager to close it so I can use my computer.
Edit: The boss fight against Nieve didn't crash, but the bullets started disappearing and we can't hit each other.
The only way to get past it is to get her down before she does that attack.
And I believe there's another fight which crashes, you've gotta keep doing that one until the boss decides not to use the skill. I forget which one it was - there's been plenty of threads about Impossible, go look at those?
