When the shared stash is about 50% full or more, it can trigger this problem.
Other times, it's just a single item in Van Helsing's inventory or in the stash. You have to search for it to find which one it is.
Sometimes you need to sell or forge any duplicates in your stash or inventory or put excess stash items in Katarina's inventory. Duplicates that were caused by a game crash are especially prone to creating problems.
Difficult cases can take hours of repetitive work, some are very resistant to being fully fixed.
The game developers have not acknowledged this issue as far as I know, so we don't know exactly what conditions cause this, or details of what steps can be taken by players.

Have you tried turning off the Steam Cloud and just letting the game use its local save files?
Since the game only keeps one backup save folder, you might like to create additional, sequentially numbered backup save folders. I put some folders on my desktop for this purpose. One folder is linked to the game's backup folder location, so I can find it quickly. The other folder contains all of my saved backups.
Saves are stored C:\ Users \ Documents \ NeocoreGames \ Van Helsing.

Very few online speed testers check the single-threaded connection, most use multi-threaded tests because they give higher speeds. I use tbb's speed test, it's UK based but should give you a decent idea of single vs. multi performance: https://www.thinkbroadband.com/speedtest Obviously, if you can find a more local test, it should give more accurate results. An in-game thing you can try that may reduce the chance of triggering the bug - don't rush exiting a location - wait a few extra seconds, 10 or so - this might allow enough time for the server to 'catch up', as well as reducing the chances of a game crash.
If you see 'Log out' bottom right of the char. select screen and you can only create up to a maximum of 6 chars, you are online and the saves and char. data are saved to, and loaded from, the online server.
My Final Cut saves are not there, how can I get my online saves on my pc?, as if I go offline theres no saves to load my toon from.
To fix the 2nd is to keep some gear on a couple mules so the main stash does not become over roughly 1/2 full. Its true that bug was never fixed but the work arounds work and we can play as desired.
The 1st is either someone fiddling with a file, or corrupted file that a steam verify files should fix.
Occassionally the game will make a dupe and when you spot two identical items, time for one of them to be transferred or trashed.
