Then there is new game + called true vault hunter mode. Starts the game over. Only thing that carries over is map quests like typhon logs and such.

Then you have mayhem which is a challenge mode. It adds modifiers to tha game that either makes it harder, more annoying, or easier depending on what random modifier you get. Full list can be found at the mayhem pedestal in Sancturary
No. Mayhem is an extra challenge mode not a traditional olaythrough difficulty. You can even enable mayhem in normal mode
Nobody plays the story twice? You have a lot to learn about borderlands games and borderlands players.
BL3 has an average of 51 hours,
BLPS had an average of 22 hours,
BL2 had an average of 56 hours.
BL1 had an average of 27 hours lol.
Clearly almost nobody gives a ♥♥♥♥ about the True Vault Hunter Mode. It's lazy and games like Destiny handle it way better.
You can always turn down the voice volume of the characters, if you know the story well enough, or just dont care.
I stopped doing side quests around level 24 and rushed main story, that way in TVHM i still have some new content to do and reasons to explore if thats your kind of thing. There are other benefits to saving side quests for TVHM but i wont get into those.
I am right about to beat the main story (on last mission while writing this) only probably explored 50-60% of the map plenty left to do, around 10-15 side quests i haven't touched, other builds id like to try, Guardian Ranks, DLC's ive yet to play (and the ones in development) i see plenty of reasons that are going to have me smashing that TVHM button.
In my opinion bl3 is all about the gunplay, if you came here just for a story, you might be disappoint
