until chaos rank 4 you can be careless about gear or your skillset, sadly also wihtout any challanginge situation. but then you will run into, "oh i shoot all my ammo into 1 enemymode". next gap is chaos rank 7, here all loottables are enabled and you can farm nearly every weapon. with chaos rank 7 you should go step by weapon to chaos 10/11 if you handel this mode solo you have finished the game. depending on your hunter and if you use some glitchedbuild you will need 300-500 hours.
Luckly there is a mayhem mode you can speed run the main story till the end and then restart from the true vault hunter mode i would say mayhem 4 is decent difficulty to start and after items started to drop you can go up till mayhem 11. And thats when the real end game begins, means you are gona farm alot of items just to be able to stand a chance againts raid bosses then you are also going to farm the raid bosses. End game is only grind tbh
For difficulty:
You can either play on easy or normal mode, and once you have finished you unlock true-vaulthunter-mode - but that is useless as it barely changes anything.
You further have the option of enabling Mayhem-mode which is the defacto way of playing the game: You can select 1 out of 10+1 difficulty-levels/modes, The higher the mode the better drop you get, starting with MH6 there are also exclusive weapons that do not drop at lower difficulty levels. basically it increases the enemy health to absurd levels and adds annoying effects like the ground under you starting to burn and dealing damage, but also increasing the Xp/loot you get.
If you want a challanging game then look at the other games in the franchise.
this happened because of game-"journalists" that were too braindead to finish a tutorial-level (and that was the reason why we got gamergate)
it still isn't hard, but it feels more satisfying because you see your bullets gradually depleting the enemy health bar, so you can "feel" each shot doing damage
meanwhile, if you use legendary weapons, it's all over in the blink of an eye (especially with non-bosses)
it's like the difference between swallowing a good meal vs actually chewing and savoring the taste
EDIT: it's like those games where you intentionally choose the slow reloading gun just because the reload is more satisfying
at the end of the day, it's not a competition we're paid to win... we play purely for pleasure, so sometimes using the weaker gun and taking longer to kill the enemy is more satisfying, well, that's the case with me anyway

Then there is the scaling of Mayhem. Since once it is max out the ennemies need to be kill a 100 times over, base damage of weapon is pretty high.
