Lost Loot, but at Max Level CAP.
Did a VERY basic google search (very lazy). Didn't seem to hit any quick tl;dr results.

What I have: Base Game + S1&S2 Season Passes
I have already progressed through most of TVHM. (In case this details matters for replication.)

Currently playing through Campaign (Pre-TVHM) with friends that just started.
We are playing in Cooperation Mode with Cartel Event: On.
I'm not max level (15 levels to MAX).

So, we play and loot is dropping at my level. I ignore most of it.
Come back to Sanctuary-3, and the Lost Loot Machine is hella full.
So, I pick that ♥♥♥♥ up.
To my absolute surprise, everything inside is scaled to Max Level Cap (no Mayhem Levels though; I'll have to test that out too).

So, I decide to ignore all Legendaries ( Legs ) I come across to test this incredible discovery.
BOOM! Max Level Cap Legs are be shat out.

Is this normal, or have I just not been paying attention to what the Lost Loot Machine is supposed to do?

If you have multiple characters at different levels, your rewards from the various BL3 events (Cartel, Broken Hearts...etc) will scale wildly. I received a lvl 53 Broken hearts reward weapon on my lvl 18 FL4K...(maybe because I have a lvl 72 Zane).

So you’re character is level 57 (72-15). You’re saying you are getting items from the lost loot machine that are 72?

I’ve never seen this happen EXCEPT when I switched characters & the lost loot was from a previous character.

PS if you joined your friend at no mayhem level, regardless of what you have set, the host determines the mayhem level & they haven’t finished the game so no mayhem.

speaking of broken hearts, where do you start it? Maurice has hearts on his head but no mission to activate, the Cartel and Haloween event worked fine

You activate it from the main menu, just like you do Revenge of the Cartels. Then while playing the game, some enemies will have hearts floating around them. Shoot those and Maurice will make a comment on your echo about 'breaking hearts for presents.'

The Lost Loot Machine is base on the order of Orange, Purple and Blue and Item Score. Base on my experience and no indication from Gearbox on how it works.

The higher the Item Score, the last one will get deleted/remove from the machine.

If you are in-game and when you pick an item/gun but decide to drop it. This item/gun WILL NOT BE PART of the Lost Loot Machine anymore. (important)

So the order of priority is Orange and Item Score. That is to say, the more Orange you have, Purple and Blue will be go to the bottom slot until it gets removed or deleted.
