why the characters has big heads?

Mayhem Modifier.

it's a mayhem modifier, one of the easy ones, it allows for easier crits but gives everyone big head mode

And you can make it go away, by changing your modifiers

I notice they sometimes have big heads outside of mayhem mode too.

Maybe it's only the cartels mission.
