Your graphics set to epic as well? Maybe higher res terrain isn't aligned with hitboxes.
The camera in the game that shows you what you see is attached to the eyes of the soldier, I forget if it's the left or the right eye. Eye level is accurate though for where the camera is positioned on the soldier.
Unlike arma, where the bullet trajectory is tied to where the firearm is pointing in game world space, bullets/projectiles in squad always go toward the dead center of your screen, before recoil.
So if your weapon is in a low ready, and you fire, that bullet is still going toward whatever you're looking at, not at the ground where the rifle is pointed.
I hope that clears some of the mechanics up.
As far as your description of what had happened, is it at all possible that there was a second enemy that shot you, and not the guy you were tunnel visioned onto?
I think most people understand this.. however because the collision effects between objects/terrain and the rifles in squad is pretty poor more often than not you find yourself shooting the top of something when close to it.. they implemented some form animation to move the rifle but it sucks ... every object seems positioned at either kneeling level or standing level .. they need to make the collision better and allow weapon resting on objects... this will sort most of the problems with shooting objects ...
as for the OP could be another person shooting you .. there is some desync at times with v12 I feel, where I can hit a guy 2 or 3 times and they still shoot back with ease.
