Whenever you see enemy, either crouch or prone as it reduces gun sway.
Don't use all your stamina, even if you don't see enemy. When you get tired, your gun sway increases. Plus you won't be able to hold your breath for long when you're about to take the shot.
And remember, the sights are above the barrel and so when you're shooting from cover, make sure the barrel is higher than the cover and not just the sights.
I'm not talking about "it's hard to aim because sway."
I mostly play games with plenty of sway and can handle those just fine. I've even handled actual rifles in a multitude of positions. I know what does and does not make marksmanship difficult. I've never experienced poorly sampled input that only lets me make relatively gross, stuttery corrections.
I am specifically talking about the fact that you can't adjust your aim in very, very fine amounts, something you need to be able to do in order to make long range shots.
Lay down with a rifle that has optics, and when you have minimized sway, make the smallest possible adjustment against a distant target. It's far too impricise to be able to actually make the appropriate adjustment. Often the smallest adjustment you can make will take you from left of the target to right of the target.
He's talking about the fact that you move your mouse as slow as possible for precision, and the game 'jumps' several degrees / moves the screen a few pixels, causing your aim to now be off target, and totally destroying any sense of precision.
Either way Tremozl described it a bit better than I was able to.

It's also recommended to put your mouse sensitivity to the max and then turn it right down to taste in the game, but I don't notice much/any difference.
Now turn your DPI up on your mouse setting in Windows let say to 2500DPI or even more. Then go to the game and config page and put the mouse sensitivity/speed as low as you can. Now aim for a mouse speed of about 10" for full character 360 turn. You might need to go adjusting both the ingame sensitivity and Windows mouse driver a few times before the result fits your taste. If you have one of those mouses with 4000+ DPI then you even get twitch shooter fast and precise aiming.
Most "pros" play with 10"+ inch and even have dedicated button to drop the DPI rate to slow the aim down.
