Go into your Manage Audio Devices window and then go under the Playback tab and manually disable everything you're not using within the playback device list. Only have your bluetooth headphones device enable.
It's a common issue for games in general to randomly select an audio device off of that playback device list.
There is also a Playback/Output selecting within the game, make sure it lists the device you're using.
The other thing id make sure it setup correctly is the properties of the device itself. Like, make sure the device is set to Stereo and not in some multichannel mode, like 7.1.
Then id make sure the Sample Rate and bit depth is set to around 44100hz at 24 bits.
If the Sample Rate is set to something extremely high, or low, this can cause issues in games that aren't mixed at those rates causing weird sounding, or totally broken audio.
