So you've done a complete / clean reinstall, including using the default settings/etc. Ok, stop looking at the game for a problem and try to find problems elsewhere.
Is your Windows up to date? Especially Internet Explorer.
Are you running anything else besides the game? In particular anything that has an overlay (Xfire, Steam's UI, similar) or anything that tries to 'hook' into the game to get information?
Are you running a lot of Javascript apps? That can mean something like keeping a browser open. Especially Mozilla / Firefox + lots of toolbars/crap running.
You could just format your hard drive and reinstall Windows... or try disabling a lot of those apps that start up automatically. Install something like CCleaner's and disable as many starting programs and try that. Or if you are at least semi-IT literate (and/or can google really well ^-^), consult Blackviper's site and prune your 'services.msc'
Also running COD in the background altered the settings and allowed the game to load properly
Anyway, FF14 hangs/crashes when it tries to enumerate your 'sound card'? Nothing in the event logs? Running COD or other games in the background will reserve or lock certain resources, forcing FF14 to use others... can you now run FF14 without having COD in the background to start?
