If you are talking about A Realm Reborn, because you bought the PS3 version you have license to play the game on PS3 only.
You have used the code for the game, you can play the game right now, but only on PS3 because that's what you bought the game for. PS3 and PS4 register the game from the PSN Store, I don't know how it works but you must have used it with your FFXIV account?
Go to the Mog Station, http://sqex.to/Msp here is the link. Log into it before you buy the game, and check if it says you have collectors edition.
This is mine, http://i.imgur.com/4bpCdEg.png I have Standard Edition of the game + it says "Win" in blue which means I play on Windows. Yours should be Collectors Edition + PS3. If it says you have Collectors Edition, you have all the bonus items no matter if you play on PC or PS3.
To play on PC, you need to buy the game on PC, but luckly you don't need to buy the collectors edition again or anything, just the normal version is fine so you have license to play on it.
So after you buy the game for PC, go back to Mog Station, click on "Your Account" option which you can see at the top of the above screenshot and you should have an option like "Add PC License"
Here a screenshot of my account. Since I have the PC version, I have options to add PS3 and PS4 options, but you should have PS4 and PC options I believe.

Sadly there is nothing what tells me that I have the collectors edition, but I guess I will just buy the normal digital version of the game, because I only have the testversion. It seems like I would have more options like those I can see on your screenshot.
On the testversion, there is just no thingy to add licenses.
Very helpful, I will type in here if it worked after buying the standard edition, if not, I will just upgrade to the collectors.

Have you linked your Square Enix account with your PSN account?
It might be best to go through Square Enix support for this since they will be able to understand everything that's going on with you account but I'll try help as best I can.
Since you don't have a PS3, you can't do it the normal way, so I think you have to do it from Square Enix's website.
But I just found out you can do it from the square enix website.
Do you have an European PSN or a North American PSN account?

Once you are there, log in and go to "My Profile" by clicking the drop down menu at the top left.
From there, click Settings and then "Linked accounts" and check if your PSN is linked and if it isn't, link it.
From there, your account should become PS3 Collectors Edition version (Check with Mog Station again), this might make it impossible for you to use the PS4 version, but if it works, then you can buy the PC version and add the license and play on PC. Hope it works this time
When you click "Your Account" at the top of Mog Station, you said you have no option to add PC license? Make sure you click "Your Account" to see all your options.
