No, my issue has not yet been resolved. I've contacted Square Enix customer support and they said they would look into it, asking for the Steam confirmation email, steam username, etc. Due to it being the holidays it's taken longer than expected. They advised me to also contact Steam support, so I suggest you do the same.
Steam specifically stated there were no refunds available for in-game purchases, but I contacted them and they seem to be looking into this.
Sorry to hear that it happened to you as well. =/ It sucks. When did yours pop up? Mine mostly likely was caused due to the Mogstation site maintenance.
The money was definitely spent, I even recieved a confirmation email from Steam saying that it's been added to Final Fantasy XIV. There's no funds in my wallet, sadly. I received both the funds added and purchase confirmation emails, so something went wrong with Steam sending it TO Square Enix, or Square Enix had issues processing due to the maintenance.
All I know is that it's lost for now. Le sigh.
Please let me know how it goes for you. I'm also gonna wait until Monday (because they're closed, while everyone else works! Hmph!) I had to send them my Steam confirmation e-mail.
Steam mentioned something about if SE really can't help be they will refund this time, however then you risk having your SE account locked for fraud. Watch them not be able to find your payment, but they do find Steam refunding fraud... Sigh. All I have to say is that Steam's customer support has been amazing and doing a better job at fixing something than SE is with their own product atm. It shouldn't take this long to sort out, holidays or not.

If you have contacted SE and sent them a ticket they will most likely ask you to provide the Steam confirmation email via attachment and email it to an email address they will enclose in their email. From there on... Well, I couldn't tell you - I'm waiting.
I'll keep you posted as well. Steam only confirmed if SE really can't assist me that they can refund it to me instead but warned me the account lock due to fraud may happen.
Imagine that... This issue not fixed, and then having to deal with an account lock issue, omg.
