While it might seem like standard movement gives you better control of the camera, I'd still recommend using legacy control mode and just setting camera-left and camera-right to the Q and E keys, the standard control mode has a "backstep" movement that moves slower than the run-towards-the-camera movement that the legacy mode has, which is important for avoiding AOEs later into the game.
Controllers tend to be where two hands become an issue. However there are some specialized controllers out there for single handed players.
Ya probably right. You can hold a controller in your lap which might help releieve any pain in his hands so I thought i'd mention it.
As I kinda stated in my other post here, the controls are VERY customisable.
With two entirely different UIs (one for controllers and one for keyboard+mouse) which can BOTH be used with EITHER a keyboard+mouse OR a controller (yes, you can use the KB+M UI with controller and vice-versa, they both have specific features to allow this), if playing with one hand with a KB+M you will probably find it easier using the controller menu (not the hot-bars) as it doesn't require use of the mouse in any way, many other features of the controls are highly customisable, such as the camera movement, targetting systems, etc.
It would also be wise to look into the game's "macro" system, which allows you to allocate 99% of the system commands to hot-keys (such as opening specific menus, targetting specific target types, etc.) which in turn allows for even more control flexibillity.
You could for example create a macro with the following:
/nt
(this targets the next hostile enemy, simmilar to the /tabkey but with specific pre-set filters) Then allocate that macro to a hot-key, lets say the hot-key that's usually assigned to the '=' key, then you could re-configure the controls to make the '=' hot-key use the Z key instead, so now you have the Z key for targetting the next on-screen enemy, which is different to the tab key that targets the next on-screen target (with customisable target filters, i.e. for targetting NPCs or alliances)
If you want to tank you could setup macros that mark mobs automatically (add me on steam and lemme know if you want an exampe of this), as a healer you can set a macro that heals the tank (regardless of who/what you have targetted at the time) etc.
The macro system is not essential but can greatly reduce the number of keystrokes required for simple (and in some cases complex) tasks.
On a topic that's sort-of-related-to-macros you can use the <mo> placeholder to "target" stuff without clicking the mouse, this could be VERY useful as you say you have the use of your right-hand index...If you have enogh use to move the mouse you could use a macro such as:
/target <mo>
Which would set your target to whatever targetable entity your mouse cursor is over (such as an NPC or an enemy), alternatively you can use something like:
/action "cure" <mo>
Which casts cure on your mouse-over target without actually switching your target.

As an off-note, this is probably THE most active game with a high number of keys required that can actually be played with one hand, for the simple fact that while it requires a huge number of keystrokes the customisation available is incredible.
