I'd say all ages. If how to train your dragon and secret of nimh is ok, this is too.
I would probably give it a T rating, similar to Earthbound because they both have similar dark themes. However, the genocide run makes it kind of hard to decide imo, that ♥♥♥♥ scared me on like horror game levels of scary.

There's a few "damns" thrown around but that's really mild and on rare occasions.

When you submit a game to ESRB, you have to show examples of the most offensive visuals in the content. So Toby would probably send videos of Toriel's fight during her "no mercy" speech,Alphys and Undyne about to kiss each other, the cutscene betweensparing Asgore and Flowey killing him, before giving a speech and then closing the game, and the speechSans gives before beginning to fight you.
Which would certainly give the game an MA rating.

The game would not be mature. It would be teen at best, and that mostly for Sans bleeding.

The esrb doesn't really care about the kind of violence in this game. There's nothing shocking to look at, it's just the implications that carry weight. For a similar comparison, look at Soul Nomad. It also has an alternate path through the game where you do horrible things, kill everyone in the world, and eventually destroy all of existance. It's rated T, and that probably has more to do with a character who cusses a lot.

Agreed, probably Teen. Maybe 10+ if that's a rating. Mostly kid-friendly but made for adults.

I don't think tthe pacifist route is any scarier than Coraline, which was still PG but is definitely not something to show your six year old. There's no foul language in the game, no nudity (well, hard to tell with monsters but you know), a couple suggestive references here and there but nothing explicit, and the worst violence is probably a character slowly melting which a kid might not figure out how to avoid.
The Genocide route has a much darker tone and drives home the point that you are the bad guy more effectively than any other game I've seen, but that's not a thing you do by accident.

Oh yeah, drug reference would definitely kick it into teen. I don't think I've ever seen an E10+ game with drug references of any kind.
