Report a translation problem
First off, the game "Crashes", so unless Toby fixed the coding so that you had to reset your SNES, which only one game did to you BTW (Xmen for the Genesis), it wouldn't work. Plus all the ♥♥♥♥ that goes on at the end would probably cause slowdown.
Maybe it can utilize the "lock on" feature to store all the data for OMEGA and whatnot.
and the crash? 'Illegal instructions' that ♥♥♥♥ and then have the player put the 2nd cart to "fix" the game.
While MOST of the upfront aspects could probably get ported or remade, some of the behind the scenes stuff (autosaves, freezes, etc) could be an issue. And some parts (Gen bosses, any boss at or after Death by Glamour) would likely suffer from signifcant slowdown.
I guess the game could just lock up and freeze, if crashing isn't possible.
"Sickos who watch" wouldn't make as much sense.
There's also the issue of a 100+ MB game being DAMN EXPENSIVE to physically produce the carts for back them. But I'll give you the benefit of the doubt and assume resolution or music quality or Game Maker bloat and unused assets could get the size reduced. Even if it takes Iwata himself pulling another "Let's add Kanto to GSC" tier compression job.
I feel the game, if officially released in the west, would have reached Western pre-SSB64 Earthbound levels of popularity and reviews. Low sales, mixed critic reviews, but silently beloved by the gamers who did experience it. Of course, that assumes it had decent coverage in Nintendo Power and was priced accordingly to SNES era standards.
As a reminder, Earthbound was NOT as big as modern internet kiddies see the game today. I mean only about 150k units sold outside of Japan (I was one of them, but eventually lent it to my cousins who misplaced it). A few positive reviews, but just as many neutral/negative reviews bashing the game's nontraditional/outwardly lighthearted art style and setting.
Of course, if Undertale launched as a $10 SNES game or as a pack-in with the system or was in some other near-free giveaway (free with NP subscription maybe?), it would probably have been a massive hit. But that seems a bit unrealistic. Perhaps releasing AFTER SMRPG might give it a little slack from "professional" critics blasted the nontraditional aspects of the setting if it were a full priced game.
But there is one major issue. Censorship would be a major issue. And I don't mean Tharja's leg. I don't mean or age alterations. I don't mean a less skimpy optional outfit in Bravely Default. Undertale would be receiving significant, game altering changes.

There's too much "oh my god!"
There's too much non-hetero relationships.
Smoking dog treats? Remember "Pub" and "beer" became "cafe" and "coffee" back then.
There's too much, as in 100% to 300%, of the "successful" suicides of FF6.
There's too much, as in 150% to 500%, of the attempted suicides of FF6.
There's one actual on-screen successful suicide (FF6's "successful" ones were only implied), though not overly graphic and requires replaying neutral ending to achieve.
This is early 90s Nintendo, the final boss of Gen would need a an edit because of no blood policy.
There would probaby be some aspects of "KILL"ing that need to be re-written as Nintendo didn't like people using the word "kill."
the game will have a battery and saves. all it needs to do is initiate a save that includes a specific flag to trigger the next event in that chain. then it can check that flag and if found, initiate it on power on / reset.
the real limitation back in the day was people didn't think to do something like that not that it wasn't possible.
Some of the graphical effects in the game however would def need to changed and possibly adding a special chip to help with said change.
