why is it that the only thing you can change in the settings menu is changing the language from english to japanese unless you're using a gamepad?

A difficulty selection does not exist, unless you mean the hard mode accessed by inputting your name as frisk, which you can only do upon a true reset if you already inputted a different name. Or you mean the genocide route, which isn't necessarily a route you can just select. If you mean something like [Easy, Normal, Hard, Expert] please know that this isn't that kind of game. This is an rpg game meant to be more of an experience than a casual game.

To answer your above question, because those are the only type of options you need. You saw a map of the controls when you first started the game. If you wish to be able to modify aspects of the game you will need to do internal editing of the game's files or download an external tool.
