He was exposed to Jenova and Mako. His mental state was in decline, so he was assessed as a failed attempt. Either because Zack escaped and rescued Cloud, or because Cloud did not meet Hojo's criteria, he is never assigned a number or given a tattoo.
There are many people who have undergone the experimental procedure who do not have tattoos, including all of the members of Soldier.
Some of the turks also might have been experimented on too (not all voluntarily either!)..vincent suggests this at one point and states he was experimented on himself by HOJO. They don't all have tattooes also. In final fantasy 7 remake, this is clearly also suggested...

NOTE: VIncent also does not have a tattoo in the game, and clearly he was a experiemented on by HOJO. Consider the other "forms" of vicent. They look similiar to some of the monsters in the game..
