The checkpoint based reload point has been the norm for a lot of games for many years now, including the 2013 Tomb Raider.
Be warned, the third title in this trilogy is already in production so most likely this will not be "fixed."
There is manual save, although I would not recommend using it on regular basis, only as a backup save.
So when you manually save the state of the game for example in slot 02, then when you load the game it will be in slot 05. First three saves on the load list are the saves made by autosave option.
These are:
- autosave;
- last camp autosave;
- copy of autosave;
- manual save slot 01.
- manual save slot 02.
Also when you load an older save the 3 autosave files are instantly overwritten. This could easily cause progression loss. That is why I do not recommend using manual saves.
I also noticed that when you die after picking up some stuff, game can load some older save, e.g. last camp save; and you will need to repick some items. In theory on each pick up game autosaves progress, but after chasing 100% game completion I got quite a few deja vu when I was looking for missed caches and docs.
If you used continue option from menu, it might have loaded last camp save - a sort of a checkpoint.
NPC will respawn after some time, e.g you leave location and then return there using fast travel.
But they numbers will be very limited, more like patrols, not a real challenge. They tend to scale up a bit so they much your exp level. I'm not sure how much game difficulty affects it. I played at Tomb Raider level and revisisting parts of map was not challenging.
Save before combat or after, never during.
