In my play experience, the quest giver never turns "green" like the other quest givers do until you are right next to him. Matter of fact, I can never see him until I am standing right in front of him.
I do know that other players have had trouble with the quest giver in the shack where you first zip down to Soviet.
This may or may not help (it always helps me), exit the game without saving to the main menu, continue the game. When I have things magically disappear, I restart the game without saving.

Yea i didn't see any one in the cave.. it seemed like the men were all in the ground because Lara even looked down to the floor :D
I had a "just plain weird" moment yesterday in the Lost City. An object "appeared" underground, but there's no underground in this area. Restarted the game & it was gone.
