I tried Misty Sun's suggestion. It did not work the first time. I then looked down as you suggested. That did the trick. Thanks.
If you dive directly from the diving board, the game will continuously glitch into a regular jump and plunge, no matter the buttons pressed.
Jump off from the rocks directly left of the board instead and it'll work as expected, having Lara enter a dive as long as the correct buttons are pressed.
You have to press and hold the button while your jump still has upward momentum and while there is a body of water directly below. Pressing too late, while you've lost upward momentum, fails and results in a normal jump and straight plunge. Pressing too early while there is still cliff-face straight below you, also fails.
This is why the dive board at the top of the long waterfall is bugged; it's positioned in such a way that it is nigh-impossible to be clear of the cliff-face and still have upward jump momentum left, because at a lower level the cliff extends outward below the board.

I know this thread is a little old. Just wanted to post that I was only able to complete the dive from the rocks just left and below the platform. None of the other suggestions here worked for me. I had no trouble at the other high dive locations.
