Any instant deaths (I think executes are included here) do this much. Were you full health beforehand?
Otherwise, as CadJuice said, it's likely another odd The Morrigan interaction (but likely not a bug).
Then it's likely just a Morrigan interaction. It probably didn't actually do that much, but did enough to kill you (some abilities also do this under certain conditions).
You're also playing The Morrigan. Were you morphed into someone while during your ultimate? Dying while morphed registers as true damage on the recap---but it wasn't actually true damage and it wasn't actually 10k damage.
You did die in a legitimate team fight and may have healed a little during the time, but in that recap it all added up.
The Morrigan and her ultimate interactions have never been stable and this is one of the things that never got fixed. You're actually controlling a "minion" when in her ultimate that will accuse the final blow of being an execute to you---because you're still a "minion" that died. The "minion" is executed when you leave the ultimate to go back to your normal self, alive or dead.
When she first launched you used to be stuck in limbo and you couldn't get control of yourself back so you were effectively a rooted spectator camera.
Tech explanation aside and TLDR: You did get the crap beat out of you, and this is just how the game interprets you dying while in Morrigan's ultimate.
"Execute damage" (9999 true) is dealt when a mechanic forcibly kills you as part of a god ability, environmental effect, or to keep gameplay from breaking.
