After that it's situational. Take 3 items (Ninja, Shogun's and Mystical are 100% core) from the following (replace bluestone and gladiator's with the last 2 items) - Winged Blade, Witchblade, Mantle of Discord, Hide of the Urchin, Spirit Robe, Frostbound Hammer, Stone of Gaia, Shifter's, Breastplate of Valour, Titan's Bane.
Example of builds:
Against Magical - Bluestone Pendant (replace with Winged Blade later) -> Ninja Tabi -> Gladiator's Shield (replace with Titan's Bane later) -> Shogun's Kusari -> Frostbound Hammer -> Bulwark of Hope
Against Physical - Bluestone Pendant (replace with Winged Blade later) -> Ninja Tabi -> Gladiator's Shield (replace with Titan's Bane) -> Mystical Mail -> Breastplate of Valour -> Frostbound Hammer -> Hide of the Lemean Lion
Against Mixed - Bluestone Pendant (replace with Mantle of Discord later) -> Ninja Tabi -> Gladiator's Shield (replace with Titan's Bane later) -> Mystical Mail -> Shogun's Kusari -> Frostbound Hammer
Of course, counter is a thing and you should watch out for it. Also, take this with a grain of salt, this is what I've done out of theorycrafting so there's bound to be some error.
3 reasons why Death's Toll is actually worse:
1 - Bluestone is better at giving you power. It's why you're capable of giving some hurt early in the game.
2 - Death's Toll is much better on gods with cleave attacks because they get a bonus. That bonus adds up when you attack as many enemies as possible. Cu doesn't have a cleave attack.
3 - You have the sustain from Gladiator's Shield anyway. GS is actually the reason you get Bluestone Pendant in the first place, you're encouraged to poke and when you do poke, you're getting the damage from bluestone. Much better than Death's Toll in this regard.
Not in comparison with Gladiator's Shield, which should give you like 2.75% HP when you hit an enemy with an ability. Death's Toll doesn't even come close in comparison. Keep in mind these are times where you have to bully hard AND sustain yourself, which contributes to the reason why Gladiator's Shield is better.
