Simon kain (spoilers)
Hello,

Spoiler question

Does the game ever confirm why or how Simon and the haruspex's father died? Was it the plague? Georgiy suspects murder and the 2nd game apparently implies that the plague was reintroduced by the 2 on purpose.

It was Oyun that killed Isidor. You learn that when you play Haruspex's route but you do get a little hint as Bachelor as well (mentions of the claw thing in Isidor's body).
Simon died of plague, yes. Simon and Isidor are drastically different characters in Pathologic 2. That said, they do cause the outbreak on purpose in 2.

Thank you! I totally forgot about him for some reason. I like how mysterious the plague is. I hope to manage to "beat" the game one day.

I don't remember Simon being in on the plan, I thought it was just Isidor that caused the second outbreak on purpose.

On the eve of their deaths, your father and Simon Kain went out into the steppe. They dug a hole. In secret.
It's vague but I'm pretty sure this line implies he knew. Georgiy says similar things about the plague as Isidor in Marble Nest as well - A plague is just a type of death to you. But to me, it's essentially an exam. And one must not run away from exams forever. Sooner or later, one must take them.
