Smiley not returning to Klamath

I saved Smiley. At the end of the 1st level. He thanks me for saving his a** and I receive 1000 exp. I even get the $100 reward from his fiance. But no Smiley, so I can't get the skin gecko perk. I've replayed it 3 times with the same result. Any clues to what I'm missing?

Are you playing the base game or with the fallout restoration pack?
If not i would play with the pack Fallout 2 has so many bugs in it and the pack helps to minimize them
