No, you don't lose the perk. I have a character with 2 bonus moves and 2 action boys at level 12.
I think you might be referring to Fallout Tactics where that is possible. In Fallout 1 and 2, if you attempt to save perks for the next level-up the perks will be lost. Unless a mod somehow changed this.

I believe Killap's Restoration Project made it possible to save perks.
Ah okay. That makes sense. So I suppose OP would be free to save perk points if thye are using the restoration project (or unofficial patch, I assume). But if not, they'd end up losing an entire perk point.

So if you install the Steam version, you can save perks.
