Same problem here. I did not see the tape on the first play through. Did not see the tape in free roam. Did not see the tape on my second play though. Pretty angry about it right now.

I just got it. After reading a bunch of posts people claiming they could do it in free roam, i figured maybe it has to do with the equipment i had on me at the time and possibly needed to reboot the game or a combination of both.
Here's what i did and here's what finally worked.

1. Full play through. Stopped by the correct location several times but the cassette was never there. Of note- There was no wood board table, just the two stumps. Completed the game with 9/10 achievements.
2. Loaded up Free Roam. Grabbed all gear from my post before heading out. Again the cassette as missing and the makeshift wood plank table was missing as well. Just the two stumps next to the chair with the note on one stump.
3. Completely shut down the game to search for what other people had to say. I booted up the game again and started a new play through but again, everything was missing just as the last two attempts noted above.

4. Completely shut down the game. Rebooted the game. Started a new Free Roam. ONLY picked up the axe from the desk, leaving everything else behind. I headed straight to the cassette location. I first went to the nearby outhouse to collect and read the note in there and kept it. Then went to the cassette location. This time the wood plank was on top of the two stumps next to the chair and the cassette was on top of the note. I first picked up the note and read it and kept it. I then picked up the cassette and once i kept it the achievement popped up for me.

Hope this helps some of you... if you can remember.

^^^ KGB-izzy's solution worked for me, for this as well as my other bugged Raccoon achievement (essentially replicated the same process, rebooted game etc except didn't grab any equipment at all at the start). I put a guide up linking back here so hopefully other people don't have to Google quite as much as I did to find this solution. Thanks !

Both Ol' Shoshone and Raccoon Carter achievements can be unlocked in free roam.

Mine failed to unlock during the campaign and multiple times in free roam.

For Ol' Shoshone load up free roam, grab the axe and head straight to the location. Before you grab the tape create a save. If the achievement doesn't work first time, create another save game after grabbing it, close the game, then re-open and continue from that save. Hopefully it'll unlock there. If not I suggest you just try to load up the save before grabbing the tape another day (after rebooting your PC) and give it another go.

The same process works for the racoon, except don't bother grabbing any equipment and head straight to the location. If you're already at the tape, I imagine you can just continue from that game to save having to walk there again. Create a save before opening the furnace. Now open the furnace and if you're lucky the achievement will unlock.

For me the racoon achievement would not unlock after continuing from a save after opening the furnace. Instead I suggest you open the save before opening the furnace. Try on different days, after rebooting your PC. Eventually it should work. Don't waste time trying to open it over and over like I did. I assume you've just got to get lucky with the servers having a good day.

Going to the note with the cassette, I took the bottle that was standing next to it and threw it away, then I immediately took the cassette and saved it, I didn't even touch the note. The achievement counted in the freeroom with one axe.

Подойдя к записке с кассетой я взял бутылку которая стояла рядом и отбросил её, потом я сразу взял кассету и сохранил её, записку даже не трогал. Достижение засчиталось во фрируме с одним топором.
