Report a translation problem
The perk description (and how it functions without Undying) is that the perk straight up deactivates once the trigger condition is fulfilled, leaving only the Exposed status effect.
You could also help everyone out by being the first to actually test the perks' interactions and report back to the wiki.
Tested with just those two hexes, Haunted Grounds moved to a new totem after being triggered.
Tested with enough hexes to use up all totems (Haunted Grounds [2], Undying [1], Ruin [1], Devour Hope [1].) After breaking Haunted Grounds, it did not re-activate.
then you combine it with noed. looks like a sweet combo then, especially if undying reveals survivour auras near totems
