Like a roguelike, if you die you start over.
But hey, completing it one one sitting makes it even more hardcore, right??? :P
lol I started it like 4 in the morning. I was totaly going to pass out.
If you want the exact same difficulty with the ability to save then play Nightmare.
EDIT: You can apparently save at the END of a mission. Save and quit. If you quit at any other point you lose your progress and you start over. Sorry for the misinformation.
And no you don't have to do the entire story in one sitting only have to do an entire level in one sitting.
If you do otherwise, the game will take you back to the first mission.
Suppose you just finished mission 1, and then saved and exited. If you continue your game from mission 2, but then quit the game half way through, the game will take you to mission 1 again.
Also once you get the last rune "Survive a death blow with slow mo and 5hp" you can pretty much use the reset trick every time you're about to die.
Then again why cheat when the whole point of the Ultra Nightmare run is for honor.
