The behaviour of the problem really points to mem leak.
There seem to be variations on what will fix it. For me, it was re-installing my graphics drivers. Sometimes a particular fix for the performance just completely stops working - like in your case, alt tabbing no longer works. In my case, re-installing GPU drivers stopped working.
I feel like there are a number of different triggers for the bug, each with different fixes.
I had success in sweeping my display drivers and re-installing them - but, again, the performance was only temporarily fixed.
Its a widespread issue, so hopefully the fix is being worked on at id.
So... no community fixes at all, other than alt tabbing, relaunching, and reinstalling GPU drivers. For some people no fix works, and they work for some - and randomly stop working.
correct, the GPU keeps fully running on doom while its alt tabbed. Nothing seems to be released.
The flaw in the idea of a memory leak is, once bugged, the issue will persist through system restarts... you can trigger the issue to go away if your lucky, but only for a time. It's a very weird bug. It's more like a cache on the HD that goes bad, but clearing all of the nvidia caches doesnt seem to fix it. Same goes for the Doom caches of data, although some people report deleting one particular doom directory fixes the issue temporarily.
That's not something you can see even on task manager.

Interesting. I would look at the config file created then, its probably bugged.
Try deleting the config file, redoing all your graphical settings and see if performance is better.
Once performance decreases, repeat the process and see if performance is back up.
Yeah, doing that has no effect. I can delete all of the config files generated, and temp doom files... same for nvidia temp and caches. None of this resets the performance.
I'm guessing its some middleware the game uses, but I have no idea
Also, for anyone reading with the issue, adjusting graphics settings is one of the biggest triggers for the performance bug... so dont adjust the settings after attemping a fix for the issue.
