Also, on a personal note, this is definitely the buggiest game I have played in my life and I've played Fallout 4. Bethesda deserves an award.
What will happen is that you'll be fine and playing on nightmare and when you go to login to your campaign the difficult will be listed in the campaign screen as I don't want to die. The best part is since you "shouldn't" be able to change the difficulty to nightmare, there's no way to change it back.
You may also notice because you'll go from getting your ass handed to you to suddenly everything being absurdly easy... which is how I noticed it happened the first time...

Screen 2: And suddenly the same campaign is "I don't want to die" (No quiero morrir in my case, but you get the idea)

Yeah man! it sucks because I thought I was doing something right in terms of my default roaming patterns during combat. Then I reloaded and poof... False hopes... I still suck at this game... :(
