What are the difficulty level options?
I've read about the ultra-nightmare. Is there something at the other end of the spectrum? Like extra wussy?

The difficulty modes are:

I'm Too Young To Die - easy mode, for wusses.
Hurt Me Plenty - normal mode, for players new to Doom or shooters.
Ultra Violence - hard mode, for old-school Doom players (I'm playing it on this mode atm)
Nightmare - really hard mode, for true Doom veterans, said to be damn hard
Ultra Nightmare - ultra-mega hard mode, for sadomasochists or psychopaths with a death wish. It has permadeath, so you get only ONE shot at completing the campaign.

How does Nightmare mode work in this game? Does it cause enemies to respawn indefinitely like in the original game or does it drain you of your health over time like in Doom 3?

To be honest, I've no idea, and I doubt there are many that do, since I imagine that only a small number of players will have finished the game by this point.

You mean, the game does not explain the differences between difficulty modes?
It does, but OP presumably didn't have the game yet.

Neither do I, hence why I am asking. I tried searching for answers online, but I keep getting articles hyping the Ultra-Nightmare mode.
Finished my Ultra-Violence run and loved it so much, I had to try Nightmare.
The enemies don't spawn indefinitely (that would ruin gore nests, lockdowns and such) or the weird Doom 3 thing. Normal health packs and armor scraps only add like +1 and an Imp fireball is 50 dmg done. It's just balls-out hard.

I am getting kicked to death on Ultra Violence, and I think I need to reduce difficulty. Five, ten, twenty, thirty, re-trys at checkpoints all the time.

I just met the first really big enemy. Beat him after about fifteen goes, dropped through a portal, and he's back. With barely any effective ammo I got no chance.

I am sick of dying and it's ruined Doom along with other stuff I ain't so keen on. Like since when was Doom a platformer; excuse me while I vomit scaling the tower.

I've never really understood people who play on the hardest difficulty and then complain that it's hard.

I beat the first 3 levels on nightmare and the difficulty felt perfect. Nice and challenging with a few deaths here and there. Then I realized there were no achievements specific to difficulty, so I started over on ultra violence to beat the game faster and get ultra nightmare unlocked.. Pretty disappointing for a lot of completionists/achievement hunters that 80% of them are multiplayer based.

They just deal more damage and are more agressive. Enemies spend more time attacking and throwing fireballs vs aiming or getting to a new position.
