Turn down the SFX to 25%, turn down the main volume to 75%, turn the music to 100%.

The music in Doom is for background effect, that in-turn sets the pace during a fire fight.

The reason it fade's in a fight, falls to you having to focus on what your doing. It's still there, but your mind shut's it down in your head. Learn to play with no sound 1st, then come back and say you can't hear it after you turn it back on.

I have never experienced that in DOOM 2016 or in other games, and it doesn't just fade in a fight. After the logos, when the music starts, it's fine for just a second and then it takes a noticeable dip in volume and stays that way for the rest of the session. Apparently Bethesda has a boner for compressing their audio to hell and back. There are guides for how to extract the music yourself.

No.

Reduce sound effects and voices volume relative to the music in the in-game settings. Then raise your OS volume until the music is at the loudness you want. You're not actually trying to make the music more than 100%, you're just trying to change the relative balance between the music and the other sounds. So lowering the non-music sounds and raising the total system volume gets you the same effect.

The "master volume" slider in game should be the volume for all the audio tracks combined, including the music, so lowering that relative to the music doesn't help you here, and I'm not sure why the other poster suggested it.

In-game "master volume" is redundant to OS volume, but if you set it low, then that limits how loud your system volume can make the game. The only real reason to set an in-game "master volume" (also sometimes called "main volume" in other games) setting lower than 100 is if you have audio from another app running in the background that you want to be louder relative to the game. Like if you're listing to a podcast while grinding, or using a separate app for voice chat; stuff like that.
