You do Have to start a new game under a new save file. You cannot change the difficulty in either nightmare mode.
As Psysomething said you have to create a new save after beating the game. (Forgive me your name is hard to rewrite, not insulting)
UN has no saves, it's a faboulous 1 run old time experience. If you die at 2 meters from the boss be ready to restart from scratch.
This was quite discussed when the game first came out, with the red text misleading players (at least mine were unlocked from the beginning too). Try to select one, just to be sure.
True for Nightmare, UN is locked until the game is completed on any difficulty.
If you see a lock next to UN, it says that Survive Into the Fire to unlock this difficulty. This means you have to complete level 6 Kadingir Sanctum at ANY difficulty, then you are free to go.
Keep in mind that if you regularly switch between Campaign and multi player, Steam client will become unstable and sometimes UN locks down again even if you have a saved completed game on Nightmare. In this case, exit Doom, Steam and restart.
