Beating Ultra Violence unlocks both of them(I've never played with I’m Too Young To Die and Hurt Me Plenty difficulties, so I don't know if they unlock Nightmare and Ultra)

Beating the game on any difficulty unlocks them both. (I beat the game on Hurt Me Plenty my first time). Nightmare/Ultra nightmare is no fun. If it's yoru first play through I advise ultra violence first.

You do Have to start a new game under a new save file. You cannot change the difficulty in either nightmare mode.

You can beat the game on "dont't hurt me please, i'm friendly" and you can unlock UN.
As Psysomething said you have to create a new save after beating the game. (Forgive me your name is hard to rewrite, not insulting)
UN has no saves, it's a faboulous 1 run old time experience. If you die at 2 meters from the boss be ready to restart from scratch.

Hmm, it's important to point out that it's possible you can play both Nightmare and Ultra-Nightmare without completing the game AT ALL. They are simply shown on the menu in a different color, but they may already be selectable

This was quite discussed when the game first came out, with the red text misleading players (at least mine were unlocked from the beginning too). Try to select one, just to be sure.

True for Nightmare, UN is locked until the game is completed on any difficulty.

I beat the game on "Hurt Me Plenty" but the game still didn't unlock the Ultra-Nightmare mode. It states that I need to beat some specific level (level 6 or smth) which I don't remember right now. I beat the whole game though. I am confused.

i have beat doom 2016 ON i'm Too Young To Die, Hurt Me Plenty, and Ultra Violence difficulties BUT when i start up doom 2016 and go to campain i only see i'm Too Young To Die, Hurt Me Plenty, and Ultra Violence difficulties. I DO NOT SEE THE OPTION for nightmare available how do i UNLOCK nightmare?

Nightmare is available by default, just highlighted with red.

If you see a lock next to UN, it says that Survive Into the Fire to unlock this difficulty. This means you have to complete level 6 Kadingir Sanctum at ANY difficulty, then you are free to go.

Keep in mind that if you regularly switch between Campaign and multi player, Steam client will become unstable and sometimes UN locks down again even if you have a saved completed game on Nightmare. In this case, exit Doom, Steam and restart.
