A question about arcade mode rating
When viewing the arcade mode scoreboard, the column farthest to the right states a given player's "rating" for a particular level and difficulty. The rating is usually a medal one managed to achieve (bronze / silver / gold). However, I noticed some people have a symbol which looks like a rune stone instead of a typical medal. My question is: how to obtain this symbol? Do any specific requirements have to be met (like: kill all enemies on the level)? Or is it simply about going past a certain threshold of points?

That's the slayer medal, basically this game's platinum medal with a hidden requirement. The best way to get that is to do REALLY good. Going fast, collecting relics and extra lives and being super efficient with your kills to increase your multiplier whilst making use of medals and overloading that bad boy for crazy points. I guess the message I'm trying to communicate here is: Don't suck.

Yeah, I got a few of those already. It seems one basically has to go over some preset hidden point threshold. Thanks for taking the time to reply! :)

is the time you need for a level also affecting the score?
And is the accuracy also affecting it?

I doubt it. Each time I got the rating was already during the level the same way I got bronze/silver/gold, meaning that you just have to beat some certain (undisclosed) point threshold. How you do it is irrelevant as long as you attain it.

It doesnt seem to be a hidden point-threshold. In Ultra-Nightmare mode I've had scores higher than someone with a slayer medal while I only had a golden medal. Then I re-ran the same map and went as fast as I could to try to preserve a high multiplier. At the end of Argent Facility (map4) I got the slayer medal right after I earned the gold medal, and right after I used the berserk on the summoner with 32x multiplier I believe it was. I didn't go out of my way to kill enemies that were in hidden areas, so it's not based on getting all kills.

I'm guessing the Slayer Medal might be a sort of achievement where you need to have already earned the gold medal AND need to kill an undisclosed number of demons with a 32x multiplier (maybe that's why it's called SLAYER medal). My second guess is that maybe throughout the whole run, if you are able to not let your multiplier hit 1x then maybe that might also give you the slayer medal near the end. Those are my guesses, if I find out something new I'll be posting here again.

So it seems I was wrong, it is based on a hidden and undisclosed fixed amount of points after all, which varies for every map and difficulty. Even though it's undisclosed, you can look up its approximate value on the leaderboard. Just pick a map and difficulty and look at what score the medals stop being golden and become Slayer Medal. For example, for Kadingir Sanctum map you'd need to reach a score of 9.7 million to get the Slayer Medal on Ultra-NIghtmare difficulty, but just 8.25 million on Nightmare difficulty for that same map. So you need a lower score to get the same medal for the lower difficulties.

But if it's strictly points how is it possible to get higher scores than someone with a slayer medal and still only get a gold ?

That only happens if they are playing at a different difficulty with respect to eachother. Lower difficulty players need to reach lower scores to get the same medal.

I just busted my butt for a few hours trying to get the gold medal in Kadingir Sanctum. I went from Silver to Slayer in one jump. The thing that seemed to make the difference was that I saved the Quad Damage for the very last minute at 32x and double barreled the crap out of everything including the two Barons. So, I believe that the slayer rating is based on getting beyond a set number of points in a certain time frame.
