I'm not saying to "git gud", unless you really want those cheevos.
Arcade Mode is meant as an additional level of challenge for those who are pretty damn good at the base game. It's meant to be above and beyond the normal game in terms of difficulty in achieving higher scores.
That's the point. If you aren't bored with the main game in terms of difficulty, if it still challenges you, then Arcade Mode isn't made for you. That's not the point of it.
You play it just like the normal game, but not stopping to smell the flowers.
Literally that's it - everything you would do to kill efficiently and quickly in the normal game will get your multiplier there and keep it there.
I had a Gold medal on my first try at the first level on Nightmare. It's really nothing different than playing the game. Slayer medal took about 7 or 8 tries. Matching the scores of some of the really good players on my friendslist though....ain't nobody got time for that.,
I'm not sure either. It has to do with combos and other things that I didn't and still don't care about. It has nothing to do with shooting skill, but memorizing aspects of the game.
Score does scale based on difficulty too - I don't know if you can do higher than silver or so on lower difficulties on the earlier levels. That could be part of it?
All you have to do is keep the multiplier meter high. Remember that pickups also help keep it high, and the generally place extra pickups in the maps between arenas to help you keep it up.
But if it's easy for you and it's not a skill factor...why not just play on UV or Nightmare and get your medals?

Just remember that some people - not just young ones - find enjoyment in the challenge. In fact, that's the main focus of why they play games.
Plus, on that "kiddies" front, games these days are generally waaaaaaaaaay easier than they used to be :D
Compared to Plutonia, everything in Doom (2016) is basically analogous to Hello Kitty Online.
