Filling the Trophy Case achievement

Another achievement, another question.

What does it exactly mean in it's description? That You have to get all the medals possible in one go? Like the Gold Medal on the first try?

And... Did anyone made a list of how many of these medals per level are? and how to get them? So that some may keep up a track.

Yea, it's one run. At the end of your run on the summary page click on the medals tab and it will show you what you need to fill the case. (14 medals total)

[Zwarte Cage]

You need to get all medals in one level, in one playthrough. This includes the Gold/Silver/Bronze relic pickups, meaning you have to wait for the relic to decay before picking it up. Your final ranking (Bronze/Silver/Gold/Slayer) doesn't matter.
