Can you change difficulty mid game?
I saw a few posts saying that you can and even seen a screenshot of a menu with the option but it's no longer there, and the console command "skill #" no longer works

Yep, via game settings in the escape menu.

I bet you play on nightmare or ultra-nightmare, then you cannot change it.
