When I lowered the settings my game stability issues virtually went away. I put all advanced graphics settings to medium, except for "Game effects" and "Shadows," which I put on low. I strongly suspect the in-game weather effects, which is controlled via the "Game effects," is lacking optimization and is the leading cause of instability for systems having dated graphics cards.
Fortunately, I think the game still looks great even on these lower graphics settings. Give it a try.
