best suggestion i have is to rewind the game to before the mission, and then follow a walkthrough to make certain everyone dies in the right order, and everyone that needs to survive is still alive
and definitely use a save mod to make saves along the way - luck
p.s. there's also some very weird dependencies or ways players have found to break this quest, seemingly related to how the Bandits at the windmill & Timmy are dealt with, although i forget the details - this is still "procedural" but related to the previous quest - just something to look at if other things fail to fix it
Ive only started playing the game again. February of last year is when I played it last, so I cant remember what I done in that playthrough.
Playing a new game now, will try that option when I see it.

There is no way that no one has died, for example, it is scripted that at the bridge, you will lose three men, EVEN IF you already drive away the enemy archers or that they didn't hit anything (even if they hit, you know full well one arrow doesn't do that much damage to guard). Basically you need all reinforcement just so they can die to scripts without any care.
The game's events are huge let down when Henry attempt to go off script. You are fighting the dev, not Runt.
TLDR; please tell Sir Radzig that there are many men (despite you probably killed all of them), well positioned archers (despite their 42-47 crap bow), etc. so that Radzig will call upon all men in the province.

if you don't do any sabotage, you have to say there are many men or you risk failing.
if you do all sabotages, you can go with fewer men
apparently arrow sabotage decreases amount of arrows carried by enemy archers (according to comments), while poison sabotage decreases their health (maybe damage too)
I sat back on the 3rd try and didnt kill anyone, and again, zero people on my side die and the quest still fails.
I dont know what to say.
Ive just got to that quest again and chose the most enemies option and same thing happens.
At the bridge no one dies. and then progressing to the next fight after the bridge no one dies there either and thats when I get the quest fail everytime.
Just my luck I guess.
Did Sir Radzig tell you to bring the message to Sir Divish so that
1. bring most of the men except minimal for garrison
2. send a message to Hanush for more men
This should be the max men you can have, being divided into three team attacking from all three entrance. One group will take out Cuman automatically; one group will take out Archers in the open field automatically.
The following are the possible intel for Radzig, depending on your scout:
0. Camp is at a ruin
1. There are many men at camp
2. There are bandits among them
3. There are cuman among them
4. archers are well positioned
5. They are building ladders
the mission can fail if a star character is killed, such as radzig,
you need to keep close to radzig to make sure he doesnt run off on his own and ends up surrounded and killed. he often runs off on his own and gets surrounded

I went to Divish with the message for more men.
For the intel I said:
Many men at camp
Bandits among them
Archers well positioned
I didnt get the option for ladders or that there are Cumans there as well.
But in my last playthrough last year, I remember I done both the sabotages and the Cumans dialogue to Radzig on top of everything else I listed there.
Yeah I noticed he runs ahead of everyone, but he never dies either. :/
I thought he and the captains are immortal, at least they were in later missions... They actually can die? Impressive.
If I can easily and unintentionally repeat it, I imagine its not just me who has the issue.
