I have an RTX 3070 and allowed GeForce Experience to set the recommended specs for this game and it set pretty much everything to ultra, but it lowered a couple of things (mostly shadow detail) that are unnecessary and resource-consuming. Also, if your GPU is powerful enough, install the 'HD Textures Pack' (free DLC) and don't forget to enable it in the game as well.
Another thing that could be a bottleneck for the game's performance is your CPU, which you didn't specify. Lastly, amount and speed of RAM could be a factor, although I've never read on these forums that RAM was the culprit for poor performance.
3090 is a great card. I suppose this means a fairly new computer, meaning RAM and such won't be the issue either.
1° Did you allow GeForce Experience to set the game's settings for you? Just to see what they recommend; you can change it anytime you want.
2° Did you install the HD Textures Pack?

Note: you need to read the install KCD Flowgraph Hook as well (bundled with OGP in that archive) to get your settings to save.
Search youtube for reviews of it and decide for yourself if it's worth trying.

my settings, for 1440p (2k) with my 1070Ti are remarkably similar to the other one posted, except my Object Distance is 50% and Object Quality is Medium (those are heavy hitters obviously).
Make sure ur Drivers are up to date.
Are you running a Laptop or PC ?
Your CPU, Memory Quantity, and HD bandwidth are also big contributors. Graphics card alone wont solve graphics optimization - u need a balanced system.
For example, I have a decent GPU but a very old CPU so I added a line to disable grass movement, while I increased draw distance.
The mod "optimized graphic preset" might not work anymore, but it is still valuable as it lets you see what lines are important to make the game run better. A lot of changes made with this mod come from an age when the game wasn't optimized at all and players had to manually tell the game to use the GPU correctly, and as such aren't necessary anymore.
To get the result I wanted, I had to manually test close to 100 settings and add them in my user.cfg. This is time consuming and you probably won't have to bother so much about it if you have a good computer.
Things like e_ShadowsMaxTexRes, e_LodFaceAreaTargetSize or e_MergedMeshesPool are usually the reason why the game doesn't look as good at it should in regard to the computer strength it uses.

I had the same suspicion. Maybe I'm naive but I don't think it's the same person because this profile is public. [deleted] has trolled this forum under several names during the past year or so, but the profile was always set to private. Also, this person genuinely replied to my posts instead of having a laugh with my replies. Lastly, this profile is 4 years old and hasn't used any aliases.
On the other hand, the profile gets a lot of negative comments and his 'reviews' are plain spam. So again, maybe you're right and I'm being naive but I give this person the benefit of the doubt. If he's laughing at me from behind his keyboard when he reads this, I don't care. If people request help or advice on this forum and they remain polite, I will try to help them, even if it eventually turns out it's just a bored person having a laugh at people wasting their time replying to their threads.

The CPU calls the draws. Great GPU but a low end 9th gen Intel and you're bottlenecked.
Obviously you're using a CPU way too slow as you have issues with the game and avoid stating the CPU.
I'm playing on med to high settings on my steam deck at 54fps, I know that doesn't have any fancy gaming GPU :(
I think my store-bought alienware (I paid lots for that, it has rainbow leds, you know) has a AMD (red coloured program shows up on settings I don't understand) 590, is that a gaming gpu? Then I have a Ryzen 5 with 16gb DDR4 3600mhz RAMs (they have cool rainbow leds too, it's fast)...
I get 120 frames on very high to ultra with that...
Can i be considered gamer by today's standards pls
