Try holding the draw sword key longer? if its a code cycle / frame rate isseu maybe.

In most cases it remains sheathed until drawn. (Some events in game also specify no weapons equipped, so check that your bow, arrows, weapon (and optionally shield) are equipped if you see no melee weapon or bow when desired - It is sensible to just glance at these when dealing with any inventory management in any case, if for no other reason than to check they have no maintenance requirement)
you cannot be sprinting, or at anything more than a walk on your horse to draw your sword. if you're in combat, anything except pressing 1 will cause you to not draw your sword, you will keep interrupting your own actions if you don't allow time for the blade to be pulled from the sheath.
moral of the story, if it's lookin dangerous, get your sword out first. you can always put it away again later, but it's pretty tough to draw it and not get murdered in the process when fighting lol...
