As far as reducing Skalitz refugee Rep - there's plenty of Skalitz refugees in the city that you can do evil to. Also, ur rep from Skalitz prologue carries to main game.
I didn't complete the quest right away, I hung onto it for a while after getting it because I was working on other things. Normally if something in a game like this (and in fact, in this game specifically) has a time-sensitive mechanic to it, you're notified. In this case it just said to go see what you can do to help them out, it didn't say when it needed to be finished. Apparently they die if you don't respond fast enough and then you don't 'fail' it but you get bad rep? So I loaded the game from when I received the quest in the first place and checked the room - sure enough, the others I could have saved are all there. I guess the quest had a time sensitive mechanic that wasn't specifically mentioned.
I put 20 hrs of gameplay between picking up that quest and completing it though, is it even worth it to restore and lose so much progress? I mean, does ensuring that those few stay alive that I missed cause other quest lines to be available?
.... and I can't for the life of me figure out why you'd throw in an insult aimed at me as the last line of your response to DargonBlak, Wonnie. Are you really just a crappy person or something?
You should not even attempt The Johanka quest line until act 2, where you should already have experience in alchemy, hunting and levelled up First Aid. Johanka seems whiny because she is trying to keep the people she has known for her entire life alive with almost no resources.
You don't miss entire quest lines, you only miss potential parts of certain quest lines, but nothing game breaking. It would only mean you'd have to find another way to progress, which is a big part of the game, imo. So either live with the consequence of your choices and maybe try it another way in another playthrough, or try to do it "perfect" the first time, which could be debatable for what even is the "perfect" way. Since this is a RPG, I'd say there is only "your" way.
To give you my opinion: no, it's not worth it. But I played several playthroughs and knew I would do so the minute I started playing this game.
Epic. You sir, are a legend. Have done her DLC ? You'll learn to hate her even more. She criticises you for knocking boots with Stephanie. Nothing worse than a hypocritical scold.
On my 4th and final playthru, I shot her in the back of the head in Skalitz. She rezzed of course, but I had nothing to do with her whatsoever after than. I let them all die of sickness in Merhojed rather than have to deal with her again.
I went to see Hermann about the guy with the broken leg, but then waited too long to go back. When I eventually went back to Sasau, buddy was lying dead half on and half off the bed. Oops.
Nobody even picked up the body ? Filthy peasants.
