My other favourite medieval game would be Mount and Blade: Warband. Very freeform experience but its only open world in the sense that you move across the world from a very zoomed out perspective. Lots of mods too. The sequel M&B2 Bannerlord is also in early access but it's not really my place to say anything about that.
If you don't mind going back to magic the older Elder Scrolls games (Morrowind and Oblivion in particular) share a similar vibe when it comes to exploration. They are far more janky, dated, not as good at telling a story, but they are more focused on action by contrast. And you can install mods like you did with Skyrim.
Maybe even try Fable Anniversary. It's world is divided into branching loading areas much like the earlier Dragon Age games. Imagine if Dark Souls was filled with a lot more life, and the combat was balanced to make you feel like a god instead of a pathetic worm, and filled with tons of humorously clear cut moral decisions. It kind of plays like that. For sure you can blast through it quicker than a typical RPG but after all this time it is still what I would consider the most charming game that I've ever played.
I found Red Dead Redemption 2 the Story not online its almost better than KCD and as good as Witcher 3 Wild Hunt. Truley brilliant. I didnt think I'd like a western but this is not just a western its epic game for exploring and discovery.

One of the few games that were able to create some tears on my eyes huh.
The gameplay on the other hand really is boring. Thats where even with some minor problems.. kcd is far far better.
Wouldnt even call it similiar.. thinking about similiar games... damn.. I wish there would be any.

Another suggestion, but to be honest it lies dormant in my library, would be the new M+B game. That one is set in the pre-medieval afaik, but it's not finished yet.

Yes, graphics are better optimized on RDR2.
I run KCD on High Textures - and i run RDR2 on Ultra Textures. 1070Ti, 2K resolution
I also strongly recommend RDR2 - it's different, and there's a learning curve - but the world is amazing. Avoid On-Line though - stick with "Story Mode" (Single-Player)
