Poisoning bandit camps
Does Bane potion lose its effects if its sitting in a simmering kettle of food for too long?

I don't think so. Save and give it a try.

Afaik, it works very well if you put it in at night and in the morning when they have breakfast they'll drop like flies.
Edit:
But I'm not sure what you consider as "too long"

I have several times dropped in a Bane potion at 10pm, fast forwarded to 7am, watched bandits eat and waited for over an hour and nothing. Other times I've dropped it in within about 3-4 hours of them eating and it worked.

Usually there's a wine flask in each bandit camp, sitting somewhere on or near a crate pile. You can poison that too, and it seems to remain potent much longer than a cooking pot.

I can concur with your observation that about 4 hours seems to be the average limit of any poison's potency in a cooking pot.

I believe that poisoning does not add to your kill count. The folks who completed the 'Merciful' achievement report that they poisoned bandit camps but none of the deaths affected the achievement.

If you want to be safe not to risk the "merciful" achievement, you could use a Lullaby potion instead of Bane potion. But I'm not sure how it would work with "kill the bandit leader"?

It could be, I think I did it once at the beginning before I could stealth kill. Knocked him out and looted the bandit leader. I'm not 100% sure if I could have taken the ear, because I then did kill him and looted again or what ever I did, not quite sure.

The spurs and masks can be pickpocketed or otherwise looted, so if you've got some Lullaby or Dollmaker potion on you, have a go and loot away. Turning them in counts as quest completed, so you don't actually have to kill the bandit/Cuman leaders.

No negative rep that I've seen, others who have the achievement may want to verify this.

It lasts or reads as poisoned until someone has eaten it. Referring to the Cauldrine in Pribyslavitz. The cauldrine at bandits camps get eaten they die .
Anything poisoned at your own whim may or may not work as its outside of a quest.
