There's only ONE person that you can't avoid kiiling (it will raise your kill rate by 1), so as long as you finish the main quest with no more than 1 kill, you'll get the achievement.
I actually caused tens of people to die in my Merciful playthrough : poisonning bandits/cumans food with Bane potion, or inficting bleeding wounds to enemies then letting them bleed out but NOT give them the killing blow will NOT count as kills ! :-)
Actually yeah, that's that : as long as you don't deal the killing blow, your kill count won't increase. So have fun finding more way for them to die while keeping your hands clean ! ;-)
As for Pious, I guess I really killed him, because otherwise there would be a prompt to kill him when standing/crouching next to him. Well, there wasn't such prompt, but the kill count is still on zero, so it should be OK. I just wanted to be sure, which I am now.. I guess :D
Anyway.. yeah, you can hold F, which will either finish them off, or stealth kill them. But when I beat up Pious, he should have been unconscious, and in that case, there would've been a prompt to finish him off, but there was only a prompt to loot him or grab the body.
