Dont think so. I wandered about quite a bit after Runt and didnt fail it. Then again, I didnt actually start the disease part until yesterday, and I trucked on through it pretty quick since I didnt have much else to do.
If you havent actually been made aware yet of the plague in-game, then no I dont think its time based. If you have, I wouldnt chance it since the plague actually becomes a side quest to do and you can fail some of those if you wait too long.
If you dont have Pestilence in your quest log at all, I would say its safe to do whatever. Im pretty sure I messed around plenty before ever starting the prisoner portion. I think it popped up for me after talking to the Baliff.
If it is there, I would complete it as soon as you can. Now that I think about it, its probably very likely you can fail it if you wait too long. (Minor spoilers) It possible to brew the wrong antidote for the plague, so its likely the prisoner can die. But he has letters on him that might point you in a different direction if he does die without telling you anything.
Up to you. Its not a difficult quest provided you know how to do alchemy. If not I dont really know how the quest ends.
